Simple Steps to Living Well Together Program

adults exercising outdoorAt the Foreign Service Benefit Plan (FSBP), Caring for Your Health Worldwide is more than just a slogan – it is the driving force behind everything we do. We actively invest in your well-being, so when you take steps to live a healthier life, we both win.

That’s why FSBP makes it easy: take the Simple Steps outlined here and earn up to $400 (you must be over 18 years of age to receive the incentives). 

The wellness rewards you earn will be applied as Wellness Incentive Fund Account credits to help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

Step 1: Complete a Health Risk Assessment  

Reward: $75 Wellness Incentive Fund Creditdoctor and patient in exam room

Completing your Health Risk Assessment (HRA) annually is an important first step to guiding your personal health goals. An HRA is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Then, you receive a personalized health summary to help you understand your health needs.

1. Go to “Aetna’s secure member website” and log on or register
2. Look for the “Stay Healthy” bar on the bottom right of the page and Select ''Discover a Healthier You.''
3. You are taken to a page that says ''For Your Information.'' Select Continue.
4. The next page will appear ''Wellness isn't far away!'' Click ''My Rewards'' to get started.
5. The next page will appear ''Wellness Rewards.'' At the bottom, select ''Work On It'' to start your Health Risk Assessment.
6. Or, call 855-406-5122 to complete the HRA by phone.

Step 2: Pass a Biometric Screening through Quest Diagnostics and have a Routine Physical Examination

Reward: $100 Wellness Incentive Fund Credit doctor taking patient's blood pressure

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time.

Get a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. 

To earn the $100 reward, you must complete a Biometric Screening and pass 3 out of 5 metabolic syndrome criteria (HDL Cholesterol, Triglycerides, Blood pressure, Waist Circumference and Glucose). 

If you do not pass 3 out of 5 metabolic syndrome criteria, you can still receive a $75 Wellness Credit by completing one of these wellness programs–Mediterranean Wellness, Health Coaching, or Dietary and Nutritional Counseling.

Register for your Biometric Screening

3. Or, print a copy of the Biometric Screening Physician Results Form to take to your physician

There are two ways to earn your incentives for the completed Healthy Actions above:

Healthy Actions (with wellness programs, if applicable) must be completed by December 1 of the calendar year. For more details about the Simple Steps to Living Well Together Program, refer to Section 5h of the FSBP Brochure

Your provider must document two controlled blood pressure readings (below 140/90) – on separate calendar year visits. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Health Coaching, or Livongo Remote Hypertension Monitoring.

Your provider must document test results showing: a 5% weight reduction, at least a 10% reduction in triglycerides, or a 5% HDL increase. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, or Dietary and Nutritional counseling.

Your provider must document laboratory results and your HbA1c must be less than 8%. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Dietary and Nutritional counseling, Diabetic Education or Training, or Livongo Remote Diabetes Monitoring.

The Plan will reach out to you if you self-identified as a tobacco user on your HRA. You must participate in the Plan’s Tobacco Cessation/E-cigarettes Program and complete at least two quit attempts that are part of the program. Each quit attempt must include four tobacco cessation/E-cigarettes counseling sessions of at least 30 minutes each. 

If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your provider for you to earn the incentive.

ITC offers:

  • One-on-one calls – participate in ongoing phone calls with a registered nurse or licensed social worker who serves as a trusted resource. To receive this individualized support, call 800-593-2354.
  • Online resources – receive digital support to better manage your health. Register or log in to Aetna’s secure member website and visit your health dashboard to get started.
  • Health Action Plans – receive customized health action plans based on your personal needs and individual preferences.

ITC Programs:

  • Social Work – connect with a licensed Social Worker who can help you locate the right resources to empower you to handle issues with confidence and clarity.
  • Pain Management – cope with chronic pain, manage opioid usage or avoid having to start opioids.
  • Compassionate Care (formerly Advance Illness Program)– manage health conditions and reduce advanced/terminal illness costs.

With this state-of-the-art communications technology, health care professionals can provide services to members who may not have been able to access services in the past. Health care professionals who provide Behavioral Health Televideo Services comply with all relevant laws in the jurisdiction where they and the member getting services are located. Scheduling an Appointment

Step 1: If you wish to book a consultation please select ‘book a video consultation’ or ‘request a phone consultation’.  Video consultations can be scheduled at a chosen time. If you choose a phone consultation the doctor will call at the next available opportunity, usually within 1 – 2 hours.

Step 2: Most fields will be pre-populated. Be sure to explain your symptoms and upload any photos of symptoms or documents such as test results (if necessary)

Step 3: If you have booked a phone consultation, there is nothing else to do. The doctor will call you on the number provided.

If you have booked a video consultation, you will need to open the app prior to the call and click ‘start video call’. From there the doctor can join you.

Appointments last 30 minutes – plenty of time to ask the questions you need.

The FSBP Clinical Team is available to support members facing health challenges. Our nurse case managers will refer you to PinnacleCare if you are suffering from certain illnesses or are in need of major procedures. AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. You will work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

The Health Coaching Program provides you and your covered dependents the opportunity to work one-on-one with a Health Coach to improve your health. A Health Coach is a health care professional who partners with you to transform your health goals into action. You can talk to a Health Coach about the following health-related matters:

  • Tobacco cessation/E-cigarettes
  • Weight management
  • Exercise
  • Nutrition
  • Stress management

How does health coaching work?

  • You talk with your Health Coach over the telephone through conveniently scheduled appointments and create a plan that is right for you to meet your health goals. Everything in the program is tailored to you.
  • You explore ways to make changes in your behavior that will last.
  • You receive written materials from your Health Coach that can help you decide where you want to go with your health and how to get there.
  • Appointments can range from 15 minutes to an hour. How long and how often you meet with your Health Coach depends on your individual needs.

Telehealth Services (Overseas Members) 

Telehealth consultations are available to members outside the 50 United States through our telehealth vendor, vHealth by Aetna. Access telehealth services 24/7/365 by web, phone, and the vHealth mobile app to receive treatment for general medicine. Treatment may include preventive care as well as treatment for acute and chronic illnesses. A provider also may give referrals and prescribe medications, when appropriate.


Note: Telehealth through vHealth is available to members with a foreign address (including APO, FPO, DPO and Pouch Mail addresses). In addition, members with a stateside address may access telehealth through vHealth by Aetna while traveling outside the 50 United States. Telehealth consultations through vHealth are available for general medicine

The Mediterranean diet is known to be one of the healthiest diets in the world. For those who want to see gains in overall well-being and appreciate a preplanned structure, try this 8-week interactive online program that includes access to a registered dietician.

You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, FSBP will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit.

The Pre-Diabetic Alert Program is focused to provide education and support for members "at risk" for developing diabetes. We will contact those individuals identified at risk and offer them the opportunity to participate in the Program. Participation is voluntary. The participant and his/her physician or other health care professional remain in charge of the participant's treatment plan.

Refer to Section 5(a), Education Classes and Programs of the FSBP Brochure for the Plan’s Diabetic Education or training benefit. FSBP covers diabetic education or training at 90% of the plan allowance for in-network (no deductible) and 70% of the plan allowance for out-of-network (no deductible).

Simple Steps to Living Well Together Program

adults exercising outdoorAt the Foreign Service Benefit Plan (FSBP), Caring for Your Health Worldwide is more than just a slogan – it is the driving force behind everything we do. We actively invest in your well-being, so when you take steps to live a healthier life, we both win.

That’s why FSBP makes it easy: take the Simple Steps outlined here and earn up to $400 (you must be over 18 years of age to receive the incentives). 

The wellness rewards you earn will be applied as Wellness Incentive Fund Account credits to help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

Step 1: Complete a Health Risk Assessment

Completing your Health Risk Assessment (HRA) annually is an important first step to guiding your personal health goals. An HRA is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Then, you receive a personalized health summary to help you understand your health needs.

1. Go to “Aetna’s secure member website” and log on or register
2. Look for the “Stay Healthy” bar on the bottom right of the page and Select ''Discover a Healthier You.''
3. You are taken to a page that says ''For Your Information.'' Select Continue.
4. The next page will appear ''Wellness isn't far away!'' Click ''My Rewards'' to get started.
5. The next page will appear ''Wellness Rewards.'' At the bottom, select ''Work On It'' to start your Health Risk Assessment.
6. Or, call 855-406-5122 to complete the HRA by phone.

Step 2: Pass a Biometric Screening through Quest Diagnostics and have a Routine Physical Examination

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time.

Get a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. 

To earn the $100 reward, you must complete a Biometric Screening and pass 3 out of 5 metabolic syndrome criteria (HDL Cholesterol, Triglycerides, Blood pressure, Waist Circumference and Glucose). 

If you do not pass 3 out of 5 metabolic syndrome criteria, you can still receive a $75 Wellness Credit by completing one of these wellness programs–Mediterranean Wellness, Health Coaching, or Dietary and Nutritional Counseling.

Healthy Actions (with wellness programs, if applicable) must be completed by December 1 of the calendar year. For more details about the Simple Steps to Living Well Together Program, refer to Section 5h of the FSBP Brochure

Your provider must document two controlled blood pressure readings (below 140/90) – on separate calendar year visits. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Health Coaching, or Livongo Remote Hypertension Monitoring.

Your provider must document test results showing: a 5% weight reduction, at least a 10% reduction in triglycerides, or a 5% HDL increase. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, or Dietary and Nutritional counseling.

Your provider must document laboratory results and your HbA1c must be less than 8%. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Dietary and Nutritional counseling, Diabetic Education or Training, or Livongo Remote Diabetes Monitoring.

The Plan will reach out to you if you self-identified as a tobacco user on your HRA. You must participate in the Plan’s Tobacco Cessation/E-cigarettes Program and complete at least two quit attempts that are part of the program. Each quit attempt must include four tobacco cessation/E-cigarettes counseling sessions of at least 30 minutes each. 

If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your provider for you to earn the incentive.

In Touch Care (ITC) Program

This combined service formerly fell under three programs, Case Management, Disease Management and Advanced Illness Program. It also broadens the scope of the Program to include a Social Work Program and a Pain Management Program.

ITC offers:

  • One-on-one calls – participate in ongoing phone calls with a registered nurse or licensed social worker who serves as a trusted resource. To receive this individualized support, call 800-593-2354.
  • Online resources – receive digital support to better manage your health. Register or log in to Aetna’s secure member website and visit your health dashboard to get started.
  • Health Action Plans – receive customized health action plans based on your personal needs and individual preferences.

ITC Programs:

  • Social Work – connect with a licensed Social Worker who can help you locate the right resources to empower you to handle issues with confidence and clarity.
  • Pain Management – cope with chronic pain, manage opioid usage or avoid having to start opioids.
  • Compassionate Care (formerly Advance Illness Program)– manage health conditions and reduce advanced/terminal illness costs.

Scheduling an Appointment

Step 1: If you wish to book a consultation please select ‘book a video consultation’ or ‘request a phone consultation’.  Video consultations can be scheduled at a chosen time. If you choose a phone consultation the doctor will call at the next available opportunity, usually within 1 – 2 hours.

Step 2: Most fields will be pre-populated. Be sure to explain your symptoms and upload any photos of symptoms or documents such as test results (if necessary)

Step 3: If you have booked a phone consultation, there is nothing else to do. The doctor will call you on the number provided.

If you have booked a video consultation, you will need to open the app prior to the call and click ‘start video call’. From there the doctor can join you.

Appointments last 30 minutes – plenty of time to ask the questions you need.

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. You will work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

In addition, members with a stateside address may access telehealth through vHealth by Aetna while traveling outside the 50 United States. You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, FSBP will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 855-482-5750 or 705-834-6782

You can also engage a registered nurse in an online chat via Aetna’s secure member website. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library in the site. 

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 855-482-5750 or 705-834-6782.

The Pre-Diabetic Alert Program is focused to provide education and support for members "at risk" for developing diabetes. We will contact those individuals identified at risk and offer them the opportunity to participate in the Program. Participation is voluntary. The participant and his/her physician or other health care professional remain in charge of the participant's treatment plan.

Refer to Section 5(a), Education Classes and Programs of the FSBP Brochure for the Plan’s Diabetic Education or training benefit. FSBP covers diabetic education or training at 90% of the plan allowance for in-network (no deductible) and 70% of the plan allowance for out-of-network (no deductible).

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart diseasetype 2 diabeteschronic pain or life events such as losing a loved one or having a baby. You will work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold.

What is the calendar year deductible?
  • For Self Only (401) enrollment, the deductible is $300 for in-network providers (including Guam) and providers outside the U.S., and $400 for out-of-network providers (including Guam).
  • For Self Plus One (403) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
  • Under a Self and Family (402) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
What's the difference between Aetna Navigator and the Member Portal?

 

Aetna’s secure member website is a system that provides members with access to information and tools regarding their health. Every enrollee and dependents ages 14 and older have their own separate login credentials for this site.

Many FSBP programs are housed in your Aetna’s secure member website account, including signing up for wellness incentives, seeing your Health Risk Assessment results, opting into electronic funds transfer (EFT), learning more about the quality and cost of your health care, and viewing your Explanations of Benefits (EOBs).


The Member Portal is a custom system for AFSPA members. It can be used to upload claims, update addresses, and enroll in and manage certain Ancillary Insurance Programs. The Member Portal is only open to the AFSPA member, not dependents. We recommend having a Member Portal account in order to allow for the fastest processing of claims.

Does the Plan have a catastrophic protection out-of-pocket maximum for deductibles, coinsurance, and copayments?

For those benefits where copayments, coinsurance or deductibles apply, we pay 100% of the Plan allowance for the rest of the calendar year after your expenses total to:

  • For Self Only enrollment $5,000 and for Self Plus One or Self and Family enrollment $7,000 for in-network providers (including Guam) and providers outside the 50 United States and when you use the Plan’s network retail pharmacy through Express Scripts (ESI), or home delivery (mail order) through the Express Scripts PharmacySM, or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam);
  • For Self Only enrollment $7,000 and for Self Plus One or Self and Family enrollment $9,000 for in- and out-of-network providers combined (including Guam) and when you use the Plan’s network retail pharmacy through Express Scripts or home delivery (mail order) through the Express Scripts PharmacySM or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam).

 

For Self Plus One and Self and Family enrollments, once any individual family member reaches the Self Only catastrophic protection out-of-pocket maximum during the calendar year, that member’s claims will no longer be subject to associated cost-sharing amounts for the rest of the year. All other family members will be required to meet the balance of the catastrophic protection out-of-pocket maximum.

Any expenses incurred that apply toward the catastrophic out-of-pocket maximum for in-network or out-of-network apply toward both in and out-of-network limits.

This catastrophic protection out-of-pocket maximum is combined for medical/surgical, mental health/substance misuse disorder, and pharmacy. There are some expenses that do not fall under this provision; see your FSBP Brochure , Section 4, Your Costs for Covered Services.
